Paint Program
Use a paint program like Gimp Portable? I'm assuming you mean you want to add text to the image rather than embed comments within the file itself (which I believe Gimp will also allow you to do).
I've never heard of a program who's express purpose is to add captions, but I wouldn't be surprised if one existed.
