As it says, it does revert back, but...
And then I am told that the highest resolution possible for me is not the highest possible

I don't know if this is a bug or not, but is kinda annoying...

The last Pathological release was 6 years ago and the project is more or less dead. At that time Vista doesn't exist. So it's maybe a Pathological bug, but it probably won't be fixed.
Possible workaround: Don't switch to fullscreen.
Is it possible to disable the full screen mode entirely? Windows Vista/7 is 23% and growing, so it's better to deal with it. Any chance it can be recompiled without the full screen option?
Sometimes, the impossible can become possible, if you're awesome!

I agree with John, it is big enough as it is and when it is fullscreen it gets a bit pixellated...

I run Vista Ultimate x64 SP 2 and the game runs without problems.
The problem with Aero accures for me, too, but Vista doesn't show any message and switches back to Aero again when the game leaves fullscreen mode.
But this is something also happening with other (commercial) games, so I think this comes from a DirectX implementation written when Aero / Vista didn't exist.
But I agree with the others that the fullscreen mode is unnecessary as the resolution isn't increased but the image is only streched over the screen which is pixeled and doesn't look that good on my 16:10 (1680x1050) monitor...

I just detected another bug, but this might be according to the game itself:
After every restart, the settings (music, sound, fullscreen on/off) are restored to their defaults.

That's a feature of the game itself: only the highscores will be saved
