The shell extension is unsupported and not recommended for use portably.

The solution was more easy than I expected; in fact once deleted en.ttt in the lang folder and renamed it.txt in en.ttt, the shell context menu appears in italian!!!!!!!!!!!
You should write this fact in some ReadMe inside the paf.exe
Moreover it MUST be supported since it made registry entry as well as 7z465.exe

The question is not whether it's supported by 7-Zip (it is) but by 7-Zip Portable. We can't support the shell extension, as it's just not portable. You're free to use it, but it will leave things behind, may slow down your computer slightly, and will launch 7-Zip in a non-portable way.
