I just recently updated from version 9 to 10. Under both versions I was unable to get the Bookmark Manager Toolbar button to appear. When I click View>Toolbar>Customize the button appears on the toolbar, but as soon as I close the pop-up, the button disappears. I tried dragging the button off and back on again and the issue remained.
I'm also missing the orange configuration button on the top left.
Any ideas what is going on or how to fix this?

My guess is that you're running a theme or an extension that is preventing the expected behavior.
You can follow standard Firefox diagnostics, but probably the easiest thing you can do is to install a clean version of FirefoxPortable (no customizations) into a slightly differently-spelled folder, such as changing the install folder to FirefoxPort or some such thing. Of course you need to close the existing FirefoxPortable, do the install, and then try the new one. If it works as expected, then you need to check out what you've modified and if you value the mod more than the orange button.

I tried installing in both a new directory and on an other USB drive and 'no joy'. I did figure out that CTRL>SHIFT>b brings up the Bookmarks Manager so at least I can work with my bookmarks.
And these were "virgin" with no attempts at configuration?

You could drag that Bookmarks menu button to the Bookmarks Toolbar, though from your description it appears like you don't actually use it. By the way, on my Win XP system the Bookmarks Toolbar was unchecked by default.
Anyway, without using the Bookmarks Toolbar, or just wishing to have the Bookmarks menu button placed somewhere else, use for that the following add-on from the Mozilla depository:
