Although PDFTK Builder is not bad, its biggest problem is that it doesn't have PDF preview.
After a very long search, I was able to find the following two free applications that have PDF preview and can do basic PDF manipulation (delete, rotate, rearrange, etc):

PDFrizator has a portable option. Not sure about PDFrider's portability but I think it should also work.
Maybe it's a good idea to add these two to the Portable Apps list as long as the authors approve.

I managed to extract PDFRider using UniExtract which is good, though I don't know how to check for changes to system or registry to make sure it's fully portable.
Nothing appeared in these folders during or after running the app:
My Documents
AppData
ProgramData
ProgramFiles

But I've already run PDFRider so that's no good now, is it? RegShot only works if an app has never been run on the system doesn't it? Does it pick up any folders added too?

You are better off using RegShot when the app has not yet been run (less chance of missing something), but it will still show any changes made to the registry even if the keys/values already exist.
It can also record file/folder changes as long as you set a base path for it look into (C:\Users\Username on Win 7 or C:\Documents and Settings\Username on Win XP are usually pretty good places to make it look).
I have installed PDFrider and copied the installation folder to couple other computers and it ran it as a limited user without any issues. So I think we can call PDFrider portable.
Overall, if I had to choose between PDFrider and PDFrizator, PDFrider seems to be a smoother package. However, the main advantage of PDFrizator is that you can visually rearrange pages, where as, in PDFrider you can't.
