When privacy settings - "Always use private browsing mode" with options "Accept cookies from sites" - see the screenshot - http://imgur.com/03SVvxt, I add site in exceptions (with option "Allow"), save changes, but after restarting the browser, there are no one exception. Is there a solution?

I see no such "Always use private browsing mode" setting within Privacy settings.
Sometimes, the impossible can become possible, if you're awesome!
It's an option which appears if you change the History DDL to "use cutom settings for history".

You can try to set the exceptions using a fresh "permissions.sqlite" database, found in the "profile" folder, and see if that helps.
With Firefox closed, just rename the existing "permissions.sqlite" file or remove it – this will cause Firefox to create it anew, automatically, upon next start.

After running several tests in new copies of Firefox PE 48.0.2, and 49.0.1, and finding no problem with keeping the exceptions, I was just about to post a suggestion that you do the same, I got an idea to check something else, and was actually able to replicate the issue you describe.
I think this could be classified as a bug. Possibly this has been already reported, I just haven't done any search regarding it, at least yet.
It is possible to get into such problem in a clean Firefox, without any add-ons installed, by setting a couple of privacy options before switching to "Always use private browsing mode", which will make them hidden.
In about:preferences#privacy "Options" panel, this is done by activating the "Clear history when Firefox closes" option, plus, activating "Site Preferences" option, under "Data", in "Settings for Clearing History" panel, accessible through the "Settings..." button.
In the background, the above settings change the status of the following two preferences, from default 'false' to user set 'true':
privacy.clearOnShutdown.siteSettings
privacy.sanitize.sanitizeOnShutdown
So, check in your Firefox, if those two prefs are toggled?
Even if the first, "privacy.clearOnShutdown.siteSettings", is set to 'true', then it's critical that the second pref, "privacy.sanitize.sanitizeOnShutdown", is set to 'false'.
The above solution, as I have mentioned, was tested in a clean Firefox. In case you use some privacy add-ons, or possibly outside cleaners, you obviously must take them into account. Plus, if you'd happen to use a "user.js" file, mind that, too ...
