www.xnview.com is home to an app that runs from any folder and can import almost any image, plus write Windows .ico files. Not sure if it is "truly" portable (no traces left behind etc.).
However, http://www.winterdrache.de/freeware/png2ico/ is home to png2ico, and the page links to a GUI for it, which can take in multiple PNGs for different resolutions of icon display then bundle them into a single file. Again, I think this is truly portable privacy-wise but the GUI needs the .NET framework, so purists won't call it portable...
XnView works portably.
The only issue is that it creates a XnView folder in c:\documents and setting\[username]\application data even if it is set to store settings etc. on the XnView appication folder.
I have written the author to ask him to fix this.
You can find XnView packaged with a launcher and settings preset for portable use HERE
You will have to delete the c:\documents and setting\[username]\application data\xnview folder manually for now.
Hopefully the author will fix it soon. The option to store in an ini file is new the folder being created was probably missed in the update.
Update: I updated the launcher so it should delete the directory if it exists.
There shouldn't be anything left behind now.

I extracted png2ico to a subfolder inside the GUI's folder, then ran it once and located the file png2ico.exe - then moved the whole lot (the GUI's folder) into my portableapps folder, refreshed app icons and it was fine...
