You mean the same icons the main menu uses, because I believe the default theme is the PortableApps.com Menu? If so, those are tango icons: http://tango.freedesktop.org

I dunno, some kind of graphics program. If you want something simple, try Paint.NET, it's not too hard to grasp and does the trick. Then you can use something like IcoFX to turn the PNGs into icons.
How do I get the individual icons? Also, I think some of PortableApps's icons follow the same format as the others.
