For some apps such as PAC and RegShot, and in making a PortableApp, you say you need a fresh install of the application, or that the computer must not contain a trace of the application.
Does this mean that it must have never been installed on the computer ever, or will a simple uninstall do, or do i have to use RegEdit and search the registry for the app name and delete the entries?
Please explain...

It means it must have never been installed or used on that PC. The best option for checking for all registry entries altered and local files changed by an app is to use a clean install of Windows within a virtual machine.
