Does the portablapps.com spec allow DLL registration (at startup) and de-registration (at shutdown) via regsvr?
In other words, can a portable app launcher take care of DLL registration?

Anything is OK, so long as it gets reverted to its original state at the end. In NSIS it's very straightforward -
