Most of these could have been taken care of by using the Application Template. (direct download)
I fixed some of your suggestions. I don't have a 128 px version of the icon and I left Other\Source\pdfMechanicPortable.ini as it is (instead of AppNamePortable.ini) because if someone wants to use it, all the they have to do is copy it to pdfMechanicPortable's root directory.

AppNamePortable.ini (as opposed to pdfMechanicPortable.ini) is a required part of the spec so a user has to manually rename the file for it to work.
This stops accidental copying of the file and the user left wondering where their splash screen went etc.
As tight as it may sound, it won't make official if there are validation errors. While the warnings and information section can (but shouldn't) be ignored, if the errors section is there when validated we won't pass on recommendation for official status.
I think its more of an not-yet-but-soon-official thing as the spec has no mention for this ini. BUt I know you're right as Chris said it should/has to be named AppNamePortable.ini.
But my apps all include the actual name because I think its nicer and I don't believe in accidental copying.
