New location for Toucan sources and bug tracker

Hello everyone,

Looks like John has got some time to get in touch with me and we've moved the sources from my SourceForge account to PortableApps' new GitHub one. So please if you have any specific bugs to report or feature requests to subit - do that at GitHub.
