NVDA 2011.1.1 has just been released; I think this is a good candidate for release when updated and checked.

I also looked at the PA.c 2.1 Guidance and found this:

Hi all,
I am not really familiar with programming but I created based on the NVDA 2010 Beta a fully working installer for NVDA 2012.1 which has no need to be in Beta stadium because of the fully portable version and the great work of the creator of the 2010 installer. I use NVDA in everyday work and so i found no problems while using it. If autostart is enabled, NVDA will start after lounching the PortableApps Louncher.
The main problem I found is that the louncher isn't really accessible so it's a little confusing because of many accessible programs.
Maybe there is a chance to integrate it into the whole apps list of PortableApps so that visually impaired people are able to install it out-of-the-box.
I'm not really familiar in how developement works, but maybe someone is reading my post and can help out to make the louncher fully accessible. I'd be glad and want to assist you in that case.

Kai, any chance of you updating this any time soon (are you even still around)?
If not I would be happy to take it over. We could really do with a few more apps in the Accessibility category and I think this would be a great candidate.
